  Accidental Death & Dismemberment - Company paid benefit available to full time employees after 3 months of employment. Coverage is equal to two times the employee's annual salary.
   
  Cafeteria Plan - Any full-time employee that has completed 3 months of service prior to January 1 will be eligible to join. This plan allows the employee to set aside pretax dollars that can be used for reimbursement of medical expenses and/or dependent care expenses.
   
 

Dental & Prescription Coverage - Regardless of the medical plan the employee chooses, our dental and prescription drug plans are also included at no additional charge. The employee must be enrolled in a medical plan to receive the dental and prescription insurance.

  1. The Dental plan pays 100% for preventative care, 80% basic services and 50% major services ($50 deductible). There is no restriction on dentists that you may visit.
  2. The Prescription drug plan allows you to get generic drugs for a $5 co-pay. Mail order is also available.
   
 

Health Insurance - Insurance is available to any full time employee after 3 months of employment. There are two types of medical insurance that are offered.

  1. Our PPO Plan has an annual deductible of $200 for a single or $400 for the family. After the deductible has been met, in-network charges are reimbursed at 90%. The member pays 10%. The monthly cost for this plan is $24.00 for a single or $140.00 for the family. The premium is paid through payroll deduction.
  2. Our HMO Plan requires the employee to choose a Primary Care Physician. The employee pays a $15 co-pay for all visits to the PCP. A referral is required for visits made to any doctor other than the PCP. The monthly cost for this plan is $27.00 for a single or $151.00 for the family. The premium is paid through payroll deduction.
   
  Life Insurance - Company paid benefit available to full time employees after 3 months of employment. Life insurance coverage that is equal to two times the employee's annual salary.
   
  Life Insurance (Voluntary) - Available to any full-time employee after 3 months of employment. This coverage is through Unum and can cover the employee, the employee's spouse and dependents. Coverage is up to five times the employee's annual salary. Premiums are paid through payroll deduction from the last check of each month.
   
  Long Term Disability - Is fully integrated with Social Security and pays 60% of the employee's monthly salary with a cap of $7,000. There is a six-month disability waiting period.
   
  Money Purchase Pension Plan - This plan is provided to the employee after he/she has completed 6 months of service. LabOne will contribute an amount equal to 7% of the employee's salary on a quarterly basis. The money will be invested in the funds of the employee's choice. There are nine investment options available.
   
  Personal Day - All full time employees will be allotted one full personal day off per calendar year. Employees must complete six months of employment with LabOne before they are eligible to use their personal day. Personal days can not be carried over into the following calendar year. If an employee does not use their personal day within the calendar year it will be forfeited. LabOne does not pay for unused personal days upon voluntary or involuntary termination of employment.
   
  Profit Sharing 401(K) Plan - This plan is available after the employee has completed 6 months of service. The plan allows the employee to set aside pretax dollars into the investments of their choice. The employee can contribute between 1-15% of their salary and LabOne will match .50 for every $1 the employee contributes up to 10%. There are ten investment options for the employee to choose from, including LabOne stock. Employees are vested after three years of employment.

  Short Term Disability - Pays up to 60% of weekly income while unable to work due to illness, injury or pregnancy. Waiting period is the later of 7 calendar days or the exhaustion of sick and vacation time.
   
  Tuition Reimbursement - LabOne offers tuition reimbursement of $150 per credit hour, up to a maximum of $2,300 per calendar year for employees who have 6 months of service prior to date class begins.
   
  Training, Conference and Seminar Opportunities - Available to employees, with Manager approval, in order to enhance current skill set.
   
  Vacation - All full-time employees accrue 6.67 hours of vacation per month. An employee is eligible to take two weeks of paid vacation after completing one year of service. During the first year of service, an employee can take one week of paid vacation after completing 6 months of service. An employee is eligible to take 3 weeks of vacation after completing 5 years of service and 4 weeks of vacation after completing 15 years of service. The maximum amount of vacation an employee can have on the books, is one week over his/her annual accrual.
